My version of a Notepad / flight plan sheet / Knee Board for the use of ATC radio communication and keeping track of your flight (plan). Originally inspired by this notepad made by yannduhamel.
The file consists of two versions: one for IFR and one for VFR. Additionally I've created and added a Quick Reference Card.
You can use it on VATSIM / BeyondATC / Pilot2ATC / SayIntentions / ... I've added also a variety of useful information on each sheet and made the colors printer friendly.

The Quick Reference Card is essentially a compact guide or cheat sheet that provides essential information a (sim) pilot might need during different phases of flight. These are used to increase situational awareness, speed up decision-making, and reduce errors —especially in high workload situations.
Features / Usage:
- color coded flow-sheme for easy communication at the departure airport and during arrival / approach.
- italic text is supposed to be said in communication
- (text in brackets) is optional / context dependent
- one IFR-version and one VFR-version
- separate Reference Card with useful information

Also, I would opt to remove/reduce some margins to get a bit more space, especially on smaller displays or printouts. Reducing the horizontal gaps might give the notes fields 1 cm more of space.

What's the "ATC name" under Clearance, Pushback/Start-up, etc? And some of them with ().

So for example an IFR clearance would follow the green line in the sheet and can look like this:
Dresden Delivery, Lufthansa 1609, A320, Gate 7, Information Tango, QNH 1006, IFR Munich, Request en-route clearance.
Keep it short.
Later you would dial in Ground frequency in order to request Push and Start following the orange line:
Dresden Ground, Lufthansa 1609, Gate 7, Request Pushback and Start-up.
Why the brackets?
At smaller Airports you will have for example Ground cover IFR clearance and Push, start and taxi. In this case (provided not too much time has elapsed), you don't have to say "... Ground" again, since he will remember the last call of you just fine. There are other circumstances as well, it's really context dependent.

nicely work... but, 2 suggestions
- remove the 7500 squawk code (on vatsim absolutely forbidden to use)... just no to inspire someone ;)
- the icao recommended abbreviation for knots is 'kt' (despite 'kn' is the standard one) .. while looking i was confused for a second... its not that you expect when reading something about speed :)
